
All players must complete the registration paperwork prior to being able to
participate in any games. In addition, the liability form and Physical forms
are required prior to try-outs/practices.
There will be a required registration meeting prior to try-outs in January.
Forms will be available at that meeting for parents.
Required items include:
(1) Forms Checklist
(2) Player Registration Form
(3) FHSAA Physical (EL2) (MUST BE FILLED OUT BY A
PHYSICIAN)
(4) FHSAA Liability Form (EL3) (both parent and player must sign)
(5) Permission Slip (available only from the front office or at the meeting)
(6) Copy of US Lacrosse Card or current proof of membership
(when you sign up for US Lacrosse, tell them group #2393409 - BHS)
(7) Fair share payment made payable to Buchholz Boys Lacrosse Boosters
Parents are also asked to provide the following:
(1) Volunteer Form (available only from the front office or at the meeting)
(2) Approval to Drive (available only from the front office or at the meeting)
(3) Copy of Driver's License and Auto Insurance card for each driving
parent
ALL PLAYERS MUST PRESENT AN ORIGINAL BIRTH
CERTIFICATE TO THE SCHOOL TO REMAIN ELIGIBLE TO
PLAY. Once the certificate is on-file, you don't have to complete this step
again in future years. Please make sure this is completed prior to the first
game and that you notify the booster president when it is done.

1. Join the Buchholz Boys Lacrosse Booster Club - contact Ken
Courington at kcour@cox.net.
2. Have the player join the US Lacrosse Organization
3. Attend the Manditory Parents Meetings in January prior to and after
tryouts.
4. Pay your "fair share" contribution and equipment fee.
5. Complete the player paperwork including a physical for the player on

6. Purchase proper protective equipment.
7. Support the player through positive reinforcement and good
sportsmanship.
8. Read the FAQ page and ask questions.
9. GET INVOLVED!!!!!!
